Before You Begin
It is important to be aware of the window period. This is the time it may take between exposure to an infection and when it could show up in a test.
* Failure to activate your kit will mean your samples CANNOT be processed. Only activate your kit when you are ready to collect your samples.
** Fill in the label with the sample collection date, your date of birth and full legal name. This label is located on the outside of the biohazard bag.
*** Collect your samples following the instructions. Place the sample tubes in the biohazard bag and seal. Place the biohazard bag in the Dropbox near the vending machine.

Collection Kit Materials
Please read these instructions carefully before collecting your samples. Before you start, make sure your kit contains the following items:

When To Collect Your Samples
Before providing your sample, first carefully read through the instructions below to familiarize yourself with how to provide a good sample for testing.
1. You should collect your samples on a Monday to Friday and return the samples on the same day.
2. Activate your LabDash account. Before collecting your samples, create a LabDash account online at LabDash.net or using the LabDash app. Follow the prompts to register. You will need to link your account to the barcode on your blood card and your urine sample tube. If you do not link your samples to your account, the lab not be able to process your samples. The information and samples you provide must be your own.
3. Fill out your information. It is important to fill out the patient identification card and blood card with your contact information, date of birth, and full legal name. This is essential for the lab to process your samples. Please do not remove any labels from the kit components as they are required for sample identification.
Urine Sample Collection
Please follow the steps in the order presented below:
4. Prepare for your urine sample collection. Do not urinate for 1 hour prior to collecting your urine sample. Do not cleanse the genital area before you collect your sample. If there is liquid in the urine sample tube, do not remove it. This Is required for sample processing.
5. Open the urine collection cup. Wash your hands with warm soapy water and dry thoroughly. Open and expand the urine collection cup by squeezing opposite corners. The cup has a plastic lining to collect urine. Please take care not to tear or puncture the lining.
6. Half fill the urine collection cup. Pass the first part of your urine into the collection cup until it is half full. Collect only the first portion of your urine stream into the collection cup. Finish urinating into the toilet.
7. Pipette from the urine collection cup into the urine sample tube. Use the pipette to transfer urine from the urine collection cup into the urine sample tube. Slowly fill the tube between the two marked lines on the side of the tube. Do not fill above or below the two marked lines or your sample cannot be processed. Never remove liquid from the urine sample tube as this may affect your test result.
8. Close the urine sample tube tightly. Screw the lid tightly onto the urine sample tube. Wash your hands in warm soapy water and dry thoroughly.
Blood Sample Collection
Please follow the steps in the order presented below:
9. Prepare for your blood sample collection. Collecting the sample after a shower or running your hand under warm waler is helpful as warm hands will make it easier to collect your sample. Ensure you are hydrated when collecting the sample. Nails that are too long can make it difficult to collect a sample. Wash your hands in warm, soapy water and dry thoroughly.
10. Select the finger for sample collection. Open the blood collection card and position It so that it will be easy to collect your sample. Select the finger you will use. The best location is the 4th finger (ring finger). It may be easier to collect the sample from your non-dominant hand. Do not pierce or collect blood from the smallest (baby/pinky) finger, or any other body part not indicated in the instructions. Please ensure you are seated when collecting your sample.
11. Prepare the lancet and pierce your finger. Take a lancet from the kit box. Twist and remove the thin tab. The lancet is now ready to use. Clean your finger with the alcohol swab provided and allow your finger to dry. Position the lancet on the right or left of your chosen fingertip. Press down firmly until you hear a click.
12. Wipe away the first drop of blood. Wipe away the first drop of blood with the gauze provided. Ensure your hand and arm are angled downwards. Firmly massage your hand and finger to form new droplets of blood. Do not touch or squeeze the fingertip.
13. Collect the sample on the blood card. As blood forms at the site of the prick, touch the blood card to the drop of blood on the indicated circle. Apply only enough blood to the card to fill the circles. Two or three heavy drops of blood per circle is plenty. Make sure the card is not soggy or overly filled.
Note: if the blood flow stops, you will need to use another finger. Repeat steps 11 and 12.
14. Let blood card completely dry. Once you have finished collecting your sample, apply the adhesive bandage to your
finger. Bend the top flap of the card up so that the sample dries while in an open position. Set the card aside until it completely dries. Avoid touching the blood card. Do not dry the blood card in direct sunlight, this may invalidate your results.
Returning Your Samples
Please follow the steps in the order presented below:
15. Place the biohazard bag into the kit box. Place the blood card in the biohazard bag. Do not remove the white pad located in the biohazard bag. Make sure patient card and blood card are fully filled out and that you have registered your sample with LabDash. Ensure you have completed the label which is located on the biohazard bag.
16. Deposit biohazard bag with your samples into the drop box. Place the biohazard bag into the dropbox located near the vending machine return samples to the drop box the same day they were collected. You will be notified via email with your results 24-48 hours after the lab receives your sample.
